Drug Interactions: What You Need to Know

Understand how drug interactions work, why polypharmacy increases risk, and how to identify potential interaction warnings in FDA adverse event data.

What Are Drug Interactions?

A drug interaction occurs when the effects of one medication are altered by the presence of another substance — whether that is another medication, a food, a supplement, or even a beverage like grapefruit juice. Interactions can make a drug less effective, increase its side effects, or create entirely new adverse effects that neither drug would cause on its own.

Drug interactions are a major concern in healthcare. According to research published in the American Journal of Medicine, drug interactions account for approximately 3 to 5 percent of all preventable adverse drug events in hospitals. For patients taking multiple medications, the risk increases significantly.

Types of Drug Interactions

These occur when two or more medications affect each other. The most common mechanisms include:

Pharmacokinetic interactions change how the body absorbs, distributes, metabolizes, or excretes a drug. For example, many drugs are metabolized by the cytochrome P450 (CYP) enzyme system in the liver. If Drug A inhibits a CYP enzyme that Drug B relies on for metabolism, Drug B’s blood levels can rise to dangerous concentrations.

Common examples include:

  • Warfarin and antibiotics — certain antibiotics inhibit warfarin metabolism, increasing bleeding risk
  • Statins and grapefruit juice — grapefruit inhibits CYP3A4, potentially increasing statin blood levels and the risk of muscle damage
  • SSRIs and MAOIs — combining these antidepressants can cause serotonin syndrome, a potentially life-threatening condition

Pharmacodynamic interactions occur when two drugs have additive or opposing effects on the same biological pathway. Taking two drugs that both lower blood pressure, for instance, can cause dangerously low blood pressure even if the individual doses are appropriate.

Drug-Food Interactions

Foods can affect drug absorption and metabolism. Beyond the well-known grapefruit interaction, examples include:

  • Vitamin K-rich foods and warfarin — leafy greens can reduce warfarin’s effectiveness
  • Dairy products and certain antibiotics — calcium can bind to tetracyclines and fluoroquinolones, reducing absorption
  • Tyramine-rich foods and MAOIs — aged cheeses and cured meats can cause dangerous blood pressure spikes

Drug-Supplement Interactions

Herbal supplements and vitamins can interact with prescription medications in ways that patients and even doctors may not anticipate:

  • St. John’s Wort induces CYP enzymes, reducing the effectiveness of many drugs including birth control pills, HIV medications, and transplant rejection drugs
  • Fish oil supplements can increase bleeding risk when combined with blood thinners
  • Calcium and iron supplements can interfere with thyroid medication absorption

Polypharmacy: The Compounding Risk

Polypharmacy — the concurrent use of multiple medications — dramatically increases the risk of drug interactions. The math is straightforward: with 2 drugs, there is 1 potential interaction pair. With 5 drugs, there are 10 pairs. With 10 drugs, there are 45 potential interaction pairs.

Elderly patients are especially vulnerable to polypharmacy risks. According to CDC data, approximately 40 percent of adults aged 65 and older take five or more prescription medications. Add over-the-counter drugs and supplements, and the interaction potential becomes substantial.

In FAERS data, reports often list multiple “concomitant” medications — drugs the patient was taking alongside the suspect drug. When you see a drug with a complex adverse event profile on our drug pages, consider that many of those events may involve patients on multiple medications.

How Drug Interactions Appear in FAERS Data

The FAERS database captures drug interactions in several ways:

Explicit interaction reports. Some reports specifically use the MedDRA term “drug interaction” to describe the adverse event. This means the reporter believed the problem was caused by the combination of drugs rather than any single medication.

Multiple suspect drugs. A single FAERS report can list more than one “suspect” drug. When a report lists multiple suspects, it often suggests the reporter believed an interaction may have contributed to the adverse event.

Concomitant medication lists. Even when only one drug is listed as the suspect, the report typically includes other medications the patient was taking. Researchers and FDA reviewers analyze these lists to identify potential interaction patterns.

On Drug Fact Check, each drug’s report data aggregates all reports where that drug was mentioned. This means the reaction counts on a drug page include events where the drug may have been interacting with other medications. The data does not isolate the effect of the drug alone.

Warning Signs of a Drug Interaction

If you are taking multiple medications, be aware of these potential signs that an interaction may be occurring:

  • Unexpected changes in drug effectiveness — a medication that was working well suddenly seems less effective, or becomes too potent
  • New side effects — symptoms that appear after adding a new medication to your regimen
  • Unusual lab results — changes in liver function, kidney function, or blood counts that coincide with medication changes
  • Symptoms of toxicity — excessive drowsiness, confusion, unusual bleeding, severe GI symptoms, or heart rhythm changes

Protecting Yourself from Harmful Interactions

Maintain a complete medication list

Keep an up-to-date list of every medication you take, including prescription drugs, over-the-counter products, vitamins, and herbal supplements. Bring this list to every medical appointment and pharmacy visit.

Use one pharmacy when possible

Pharmacists are trained to check for drug interactions. If all your prescriptions are filled at the same pharmacy, the system can flag potential interactions automatically. Using multiple pharmacies increases the risk that an interaction will be missed.

Tell every healthcare provider about all your medications

Different specialists may not know what other doctors have prescribed. Always inform each provider about your complete medication list, including drugs prescribed by other doctors.

Be cautious with over-the-counter drugs

Just because a drug is available without a prescription does not mean it is free of interaction risks. NSAIDs like ibuprofen, antihistamines like diphenhydramine, and antacids can all interact with prescription medications.

Ask your pharmacist

Pharmacists are drug interaction experts. If you have any questions about whether two medications can be taken together safely, your pharmacist is often the best person to ask.

Check for interactions before starting new medications

Drug Interactions and the FDA

When the FDA identifies a clinically significant drug interaction through FAERS data or other sources, they may take several actions:

  • Label changes — requiring the drug manufacturer to add interaction warnings to the prescribing information
  • Drug Safety Communications — issuing public alerts about newly identified interactions
  • Boxed warnings — for the most serious interactions, requiring a prominent warning on the drug’s label
  • Contraindication updates — formally stating that the drug should not be used with certain other medications

These regulatory actions protect patients, but they rely on adverse event reports being submitted. If you experience what you believe may be a drug interaction, reporting it to the FDA contributes to this safety system.
