Why Some Reactions Appear Everywhere
If you browse the drug pages on Drug Fact Check, you will notice that certain adverse reaction terms appear repeatedly across many different medications. Terms like “drug ineffective,” “nausea,” “fatigue,” and “pain” show up in the top reactions for drugs as different as antidepressants, blood pressure medications, and cancer treatments.
This guide explains the most commonly reported adverse event terms in the FAERS database, what they actually mean, and why they appear so frequently.
Drug Ineffective
“Drug ineffective” is consistently one of the most reported adverse events in the entire FAERS database. It appears in the top reactions for a wide range of medications. This term does not describe a side effect in the traditional sense. Instead, it means the reporter indicated that the drug did not achieve its intended therapeutic effect.
There are many reasons a drug might be reported as ineffective:
- The patient’s condition did not improve
- The dose was insufficient for that particular patient
- The patient was not taking the medication correctly
- Drug interactions reduced the medication’s effectiveness
- The diagnosis may have been incorrect
“Drug ineffective” reports are valuable to the FDA because a pattern of ineffectiveness could signal manufacturing problems, formulation changes, or counterfeit products entering the supply chain.
Nausea
Nausea is one of the most universally reported adverse events because it is a common side effect of many drug classes. The gastrointestinal system is sensitive to foreign substances, and many medications — particularly those taken orally — can irritate the stomach lining or affect the brain’s nausea center.
Drug classes where nausea is especially common include:
- Opioid analgesics — morphine, oxycodone, hydrocodone
- Chemotherapy agents — nearly all cancer drugs
- Antibiotics — particularly macrolides and fluoroquinolones
- SSRIs — selective serotonin reuptake inhibitors like sertraline and fluoxetine
- NSAIDs — ibuprofen, naproxen (especially with long-term use)
When you see nausea reported for a drug, it is almost always a known side effect listed on the drug’s label. The number of nausea reports typically correlates with how widely the drug is prescribed.
Headache
Like nausea, headache is a non-specific symptom that can be caused by many medications or by the underlying condition being treated. Headache appears frequently in FAERS reports for blood pressure medications (where blood pressure changes can trigger headaches), vasodilators (like nitrates), hormonal medications, and many others.
Headache is also a common symptom of medication withdrawal. Patients who stop taking certain drugs abruptly — particularly caffeine-containing products, opioids, or antidepressants — often experience rebound headaches.
Fatigue
Fatigue is reported across virtually all drug classes. Many medications cause drowsiness or tiredness as a direct pharmacological effect — antihistamines, benzodiazepines, and beta-blockers are well-known examples. But fatigue is also a symptom of the conditions these drugs treat, making it difficult to determine whether the drug or the disease is responsible.
In FAERS reports, “fatigue” often overlaps with related terms like “asthenia” (weakness), “malaise” (general discomfort), and “somnolence” (sleepiness). Together, these fatigue-related terms account for a significant portion of all adverse event reports.
Pain
“Pain” as a MedDRA Preferred Term is intentionally broad. It can refer to any type of pain the patient experienced while taking the medication. More specific pain terms like “back pain,” “abdominal pain,” “chest pain,” and “musculoskeletal pain” are coded separately.
When you see generic “pain” in a drug’s reaction list, it usually means the reporter did not specify the location or character of the pain, or the pain was generalized. For drugs used to treat pain conditions (like opioids or NSAIDs), “pain” reports may indicate treatment failure rather than a side effect.
Death
The term “death” in FAERS reports requires especially careful interpretation. A death report means the patient died while taking or recently after taking the medication. It absolutely does not mean the medication caused the death.
Consider that patients taking chemotherapy drugs, HIV medications, or treatments for heart failure are already seriously ill. A significant number of these patients will die from their underlying disease regardless of which medication they take. FAERS captures all of these deaths if the patient was taking the drug at the time.
For widely used medications taken by generally healthy populations (like statins or blood pressure drugs), death reports are typically a very small percentage of total reports and may be entirely coincidental.

Off-Label Use
“Off-label use” is a reported event that indicates the drug was used for a purpose not specifically approved by the FDA. Off-label prescribing is legal and very common — some studies estimate that up to 20% of prescriptions are for off-label uses. It is reported as an adverse event when the reporter wants to note that the drug was being used outside its approved indication.
This term does not describe a side effect. It is context about how the drug was being used when the adverse event occurred.
Drug Interaction
Reports of “drug interaction” indicate that the reporter believed the adverse event was caused by an interaction between two or more medications the patient was taking simultaneously. Dizziness
Dizziness is a common adverse event for medications that affect the cardiovascular system (blood pressure drugs, diuretics), the central nervous system (sedatives, antiepileptics), and the vestibular system (certain antibiotics). It is also a frequent symptom of starting or stopping medications, as the body adjusts to changes in drug levels.
In elderly patients, drug-induced dizziness is a significant safety concern because it increases fall risk. If you experience dizziness while taking a medication, it is worth discussing with your healthcare provider.
Diarrhoea / Diarrhea
Gastrointestinal disturbances, particularly diarrhea, are among the most frequently reported adverse events. Antibiotics are notorious for causing diarrhea because they disrupt the gut microbiome. Metformin, one of the most widely prescribed diabetes drugs, also commonly causes GI symptoms.
Note that FAERS uses MedDRA terminology, which follows British English spelling conventions — so you will see “diarrhoea” rather than “diarrhea” in the data.
Rash
Skin reactions, broadly categorized under terms like “rash,” “pruritus” (itching), and “urticaria” (hives), are common adverse events across many drug classes. Rashes can range from mild and self-limiting to severe and life-threatening (as in Stevens-Johnson syndrome or toxic epidermal necrolysis).
Allergic and hypersensitivity reactions are a frequent cause of drug-related rashes. Antibiotics (particularly penicillins and sulfonamides), antiepileptics, and NSAIDs are among the most common culprits.
How to Use This Information
Understanding what common adverse event terms mean helps you interpret the data on this site more accurately. When you see “drug ineffective” as the top reported reaction for a medication, you now know this is about efficacy, not side effects. When you see “death” in the report counts, you know this does not imply causation.
